Definitions

We use common language and definitions to help you understand important terms throughout the Privacy Statement and Terms and Conditions.

“Chicago Genetic Consultants, LLC”, “We”, “Our” or “Us” — Chicago Genetic Consultants, LLC includes our telehealth practice.
“Chicago Genetic Consultants, LLC Services” — Collectively refers to all of the following services that are owned by, operated and/or managed by or for Chicago Genetic Consultants, LLC:
- “Chicago Genetic Consultants, LLC Websites” — Our websites include chicagogenetics.com.
- “Chicago Genetic Consultants, LLC Patient Portals” — Our patient portals allow patients to access their schedule visits.
- “Chicago Genetic Consultants, LLC Additional Services” — We also provide services, digital offerings and tools, including, but not limited to: customer service; technical support; QR codes; search tools; employee recruitment tools; clinician profiles; location listings; digital signage; and subscriptions, reminders, alerts and marketing messages using direct mail, email and SMS (text messages).
“You” or “Your” — You, regardless of being an individual, a group or an organization, and regardless of whether you are using Chicago Genetic Consultants, LLC Services on behalf of yourself or someone else.
“Your information” — Collectively refers to an individual’s personally identifiable information (“PII”), and/or protected health information (“PHI”).
Personally identifiable information (“PII”) — An individual’s first name or first initial and last name in combination with any one or more of the following data elements: Social Security number; driver’s license or state identification card number; account number, credit card number or account information related to a financial account; medical information; health insurance information; and/or unique biometric data generated; OR, a username or email address, in combination with information that would permit access to an online account. Protected health information (“PHI”) — Any information regarding an individual’s medical history, mental or physical condition, or medical treatment or diagnosis by a healthcare professional, including such information provided to a website or mobile application.

About your protected health information (“PHI”)

Unless otherwise specified, please note that this Privacy Statement only applies to personal information you provide through your use of the Chicago Genetic Consultants, LLC Services. As otherwise not legally prohibited, Chicago Genetic Consultants, LLC and third-parties may collect personally identifiable information (“PII”) that you choose to provide to the Chicago Genetic Consultants, LLC Services. When necessary, you will be prompted to accept additional terms and conditions when using Chicago Genetic Consultants, LLC Patient Portals via a web browser (see “Chicago Genetic Consultants, LLC Patient Portals and what we collect”).

This Privacy Statement does not apply to any protected health information (“PHI”) gathered via the Chicago Genetic Consultants, LLC Services. Rather, Chicago Genetic Consultants, LLC’s Notice of Privacy Practices will govern Chicago Genetic Consultants, LLC’s use and disclosure of your PHI. The Notice of Privacy Practices describes how information about Chicago Genetic Consultants, LLC’s patients may be used and disclosed. Please review it carefully.

Acceptance of the Chicago Genetic Consultants, LLC Privacy Statement

By accessing the Chicago Genetic Consultants, LLC Services and/or using any of the features or services provided on the Chicago Genetic Consultants, LLC Services, you signify your acceptance of the Chicago Genetic Consultants, LLC Privacy Statement and consent to the information collection and use practices described herein. If you do not consent or otherwise do not agree with the Chicago Genetic Consultants, LLC Privacy Statement, please do not use or access the Chicago Genetic Consultants, LLC Services.

Information we collect & how it is collected

Chicago Genetic Consultants, LLC may collect your PII through the Chicago Genetic Consultants, LLC Services. We may collect PII actively (i.e., by collecting information you voluntarily submit to Chicago Genetic Consultants, LLC) and passively (i.e., by collecting information automatically gathered from your computer).

Personally identifiable information (“PII”)

In some instances, Chicago Genetic Consultants, LLC, through the Chicago Genetic Consultants, LLC Services, and/or a third-party vendor or other third parties may collect PII (including but not limited to email address, Internet Protocol (IP) addresses, name, phone number, and other information as defined under the Illinois Personal Information Protection Act). This information may be collected when accessing the Chicago Genetic Consultants, LLC Services or when volunteered by the user (e.g., when you create a patient portal account, when you schedule an appointment, when you submit a web form or when you answering a question prompt posed by a chatbot tool). If you send Chicago Genetic Consultants, LLC an email request or participate in our forums, Chicago Genetic Consultants, LLC may store that information as well, including any PII included in a post, message or email, as well as any PII included in any attachments you may include.

When applicable, you may also use the Chicago Genetic Consultants, LLC Services to pay a bill (“Transactions”). To process a Transaction, Chicago Genetic Consultants, LLC or one of our third-party vendors (“Payment Processors”) may collect PII, such as your banking information, like your name, billing address, credit or debit card number, email address, telephone number and/or email address. We or Chicago Genetic Consultants, LLC’s Payment Processors will process your credit card payment using established encryption techniques that protect your information.

If a Payment Processor processes your Transaction, Chicago Genetic Consultants, LLC encourages you to read such Payment Processors’ privacy statement or policy and terms of use before entering any PII. Chicago Genetic Consultants, LLC may not receive complete payment information for a third-party vendor-processed Transaction, but Chicago Genetic Consultants, LLC’s Payment Processor will provide Chicago Genetic Consultants, LLC with some PII upon completion of the Transaction, such as your name, billing address, email address, telephone number, amount paid, how you heard about Chicago Genetic Consultants, LLC, and the last four digits of your credit card number, the latter of which Chicago Genetic Consultants, LLC uses for verification and/or customer service purposes.

Platform information

When you visit Chicago Genetic Consultants, LLC Services, Chicago Genetic Consultants, LLC’s servers automatically record certain information about the device (e.g., a computer or mobile device) you used to access the Chicago Genetic Consultants, LLC Services. Text messaging communications

When you sign up to receive text messages from us via the Chicago Genetic Consultants, LLC Services, we collect your name and phone number, information about your browser, information about the page you signed up on, and any other additional information you may provide to us. Employment, fellowship and grant applications

If you contact us in relation to your employment, we will use your personal information to consider you for current and future employment and to contact you with respect to such opportunities at Chicago Genetic Consultants, LLC that you have expressed an interest in. In connection with your application we may collect personal identifiers, such as your name, postal address, email address, or other similar identifiers; and professional or employment-related information about you, such as current or past job history or performance evaluations.

Chicago Genetic Consultants, LLC Patient Portals & what information we collect

This section titled “Chicago Genetic Consultants, LLC Patient Portals, Other HH Mobile Apps & what information we collect” only applies to an individual’s access and use of the Chicago Genetic Consultants, LLC Patient Portals.

To provide patients and visitors with a useful and personalized patient experience, the Chicago Genetic Consultants, LLC Patient Portals may require access to additional features and/or information stored on your web browser or smartphone. The following describes the ways Chicago Genetic Consultants, LLC may access, collect, use and share your PII and/or PHI:

Location: Chicago Genetic Consultants, LLC Patient Portals do not collect or store your location data or share it with third parties without your consent. Location data can be used by mobile app features, such as sending notifications to Chicago Genetic Consultants, LLC’s registration staff when you’ve arrived for an appointment or suggesting healthcare providers or locations that are nearby.
Chicago Genetic Consultants, LLC Patient Portals may offer location-based check-in for in-person appointments or allow you to find healthcare providers near you. The first time you try to use any features that use your location, you will be asked for your consent to share your device’s location information within the app. Your location information will be used only if you give your consent, and your consent can always be revoked by changing your privacy permissions. You do not have to provide consent if you do not want to allow the Chicago Genetic Consultants, LLC Patient Portals to use your location.
Camera and photo library: Chicago Genetic Consultants, LLC Patient Portals may use your device’s camera feature, or require access to your photo library, to let you take and/or share photos or to capture and stream video for telehealth visits. If you choose to allow the Chicago Genetic Consultants, LLC Patient Portals to interact with your photo library, you can upload photos to personalize an account or provide health record attachments and these photos may be accessed by the healthcare providers.
Microphone: If you choose to allow the Chicago Genetic Consultants, LLC Patient Portals to use your device’s microphone, you can use your microphone to navigate the Chicago Genetic Consultants, LLC Patient Portals and/or can be used to capture audio for telehealth visits. The Chicago Genetic Consultants, LLC Patient Portals do not store your audio data.
Storage: Chicago Genetic Consultants, LLC Patient Portals may store files you upload from your device’s storage to your medical record. The Chicago Genetic Consultants, LLC Patient Portals may access your device’s storage to read and write files you select in the application. These files may be used as file attachments that are sent to your healthcare provider or they may be created from file attachments sent to you from your healthcare provider.
Bluetooth: Chicago Genetic Consultants, LLC Patient Portals may access your device’s Bluetooth feature. Bluetooth may be used to detect other devices nearby that can help with your appointment arrival. The Chicago Genetic Consultants, LLC Patient Portals do not store your Bluetooth data.
Phone: Chicago Genetic Consultants, LLC Patient Portals may use your smartphone to call phone numbers selected by you in the application. The Chicago Genetic Consultants, LLC Patient Portals will not collect, store or share your call history or any other call data.
Telehealth: Chicago Genetic Consultants, LLC Patient Portals may allow you to conduct a telehealth appointment with your healthcare providers, such as a video visit or a phone appointment that may be subject to additional terms and conditions for you to review and accept. Always confirm with your provider if they provide telehealth services. The Chicago Genetic Consultants, LLC Patient Portals only provide the technical capabilities for telehealth appointments to happen. Chicago Genetic Consultants, LLC Patient Portals do not capture any health information about you that is discussed or exchanged with your provider during live telehealth appointment sessions.

In addition to giving you a notice of technology changes, if subsequent versions of Chicago Genetic Consultants, LLC Patient Portals collect new types of PHI and/or PII, you may be notified by a pop-up message of these changes on Chicago Genetic Consultants, LLC Patient Portals with an explanation of why new data is being collected.

Chicago Genetic Consultants, LLC Patient Portals may interact with your PII or PHI to provide you with certain features, such as video visits or mobile appointment check-in. Please refer to Chicago Genetic Consultants, LLC’s Notice of Privacy Practices to learn how Chicago Genetic Consultants, LLC handles your PHI. The first time you try to use any of these features, you will be asked for your consent within the Patient Portal and you may only use a feature if you give your consent. You do not have to provide consent if you do not want to allow the Chicago Genetic Consultants, LLC Patient Portals to interact with your data as requested.

The Chicago Genetic Consultants, LLC Patient Portals are developed by or in conjunction with therapyappointment; please refer to the for more detailed information about therapyappointment privacy policy limited ways therapyappointment may interact with your information to make your use of the Chicago Genetic Consultants, LLC Patient Portals possible.

Privacy of children

Because Chicago Genetic Consultants, LLC is committed to protecting the privacy of children, the Chicago Genetic Consultants, LLC Services are not intended for use by or directed at children under the age of 13. We do not knowingly nor intentionally collect PII from anyone under 13 years of age via Chicago Genetic Consultants, LLC Services. We encourage parents and guardians to take an active role in their children’s online and mobile activities and interests. If we are made aware that we have received such information, or any information in violation of our policy, we will use reasonable efforts to locate and remove that information from our records.

How we process your PII

Chicago Genetic Consultants, LLC is committed to processing your PII in accordance with applicable data privacy laws and with transparency and fairness. We will only process your PII if:
